The Volusia County Health Department is a Vaccine for Children (VFC) vaccine
provider.
- We give all shots necessary for Childcare centers and schools.
- DOH Form 680 ("Blue Cards") are available upon request.
- DOH Form 681 can be obtained through our Immunization Department nurse

Tuberculosis Screenings/PPD Skin Test
All of our clinic sites administer the Tuberculosis Screenings/PPD Skin Tests. They are performed Monday thru Wednesdays,
and Fridays * (see note below about tests on Friday). The tests are not performed on Thursdays because of the need to return to have the results read
by a nurse within 48-72 hours.
* The Tuberculosis Screenings/PPD Skin Tests are not performed on Fridays when the following Monday is a holiday observed by the Volusia County Health Department.

- Full and complete shot record
- Written consent from parent or guardian if parent is not present. The written document must contain:
- current date
- child's name
- child's date of birth
- parent's signature
- parent's contact phone number

Please note we are closed for the following holidays - New Year's Day, Martin Luther King Day, Memorial Day,
Independence Day, Labor Day, Veteran's Day, Thanksgiving Day, Friday after Thanksgiving and Christmas Day.
