The Volusia County Health Department is a Vaccine for Children (VFC) vaccine
provider. We give all shots necessary for Childcare centers and schools.
DOH Form 680 ("Blue Cards") are available upon request.
Religious Exemptions for immunizations (DOH Form 681)
can be obtained through our Immunization Department nurse
